Child Support Owed

Under Section 51.70 (a) (8) of Title 22 of the Code of Federal Regulations, a passport, except for direct return to the United States, will not be issued in any case in which the applicant has been certified by the Secretary of Health and Human Services as notified by a state agency to be in arrears of child support in an amount exceeding $2,500.00.

Your passport will be revoked if you fall under this regulation, including when you apply for additional pages, seek a renewal or process a Report of Birth application.

Adding Pages to Your Passport

For Customers Coming to Tokyo or Naha

This service is provided by appointment only. Please make an appointment online.

If your full-validity passport has not yet expired, but has no blank pages left, we can add additional pages for you free of charge. You can apply by mail (see instructions below). Processing time is only a few days, except in Naha where it may take one-two weeks.

We can only add two sets of pages; after that, you will need to replace the passport even if it has not already expired.

We cannot add additional pages to a limited validity passport. Limited passports have a duration of less than 10 years (for adults) or less than 5 years (for children). If you run out of blank pages in a limited validity passport, you must apply for a new, full validity passport. Details on how to apply are here.

Application Form for More Pages
Please complete an application form (DS-4085). Note that the online version of this form includes instructions for use in the U.S. which do not apply to your application in Japan.

This form must be completed, and signed by the passport holder him/herself, regardless of whether you seek additional pages by mail or in person.

There is no fee for this service.

Please send us your passport, the completed, required form (DS-4085), a note requesting additional pages, and a pre-purchased "Expack 500" envelope, available at Japanese post offices and most convenience stores for 500 yen. The "Expack 500" provides express return of your documents and includes a barcode so you can track your documents as they make their way through the Japanese postal system.

Persons using an APO address should include a regular legal size or larger self-addressed envelope marked "MPS", except on Okinawa where things will be returned to you by military courier. Please note: persons using an APO address cannot use the "Expack 500".

Please mail your materials to the "PASSPORT UNIT" serving your part of Japan.

Military members on Okinawa should send their materials to the Consulate in Naha by military courier; all other APO users please send your materials to "PASSPORT UNIT", U.S. Embassy in Tokyo. Our addresses are here.

Changing Your Name in Your Passport

For Customers Coming to Tokyo or Naha

This service is provided by appointment only. Please make an appointment online.

If you would like to change your name in your passport to reflect a marriage or legal name change, you will need to apply for a new "replacement" passport.

Here's what you'll need:

Application Form
If you apply to have your name changed within one year of your current passport's issuance date (i.e. your current passport is less than one year old), you should submit a "U.S. Passport Re-Application Form" (DS 5504), together with the passport that needs the change, the documentation required (see below) and two (2) 2 inch by 2 inch (5 cm x 5 cm) passport photos.

Note that the online version of the application form includes instructions on the back side written for use in the U.S. which do not apply to your application in Japan.

If you apply to have your name changed one year or later after your current passport's issuance date (i.e your current passport is more than one year old), you will need to submit an "Application for Passport By Mail"(DS-82) and pay the full fee for a new passport: $75 for adults, $85 for children. You will also need to send us the passport that needs the change, the documentation required (see below), and two new 2 inch by 2 inch (5 cm x 5 cm) passport photos.

Documentation
You'll need to show us legal documentation supporting any requested change. For example, your original marriage certificate showing your new spouse's name or an original court order showing a legal name change.We cannot accept copies, and we reserve the right to request additional documentation.

By Mail

Correcting Printing Errors in Your Passport

If your passport contains incorrect data about you (such as a mispelled name, incorrect date of birth or incorrect gender) because of a printing error made by Passport Services, you should act as quickly as possible to have the error corrected.

If you're applying to correct an error, you should submit a "U.S. Passport Re-Application Form" (DS-5504), together with the passport that needs the change and two (2) 2 inch by 2 inch (5 cm x 5 cm) passport photos.

Note that the online version of the application form includes instructions on the back side written for use in the U.S. which do not apply to your application in Japan.

To correct an error in your passport you will need to provide original (no photo copies will be accepted) documentation showing that the information in the passport is incorrect. This documentation might include your birth certificate, your marriage certificate, appropriate court orders and/or other forms of government-issued identification.

Of course there will be no fee to correct an error if the fault lies with Passport Services.

Extending a Limited Validity Passport

For Customers Coming to Tokyo or Naha

In the past we issued two types of limited validity U.S. passports; those that could be extended with authorization from the Department of State and those that could not. Passports can no longer be extended, and persons seeking extensions will now be required to apply for a "replacement passport".

If, in the back of your passport, there is a message stating that your passport may be extended you will now need to apply for a replacement passport using the "Application for Passport" (DS-11) and pay the full fee for a new passport ($100 for adults, $85 for minors). You will also need to send us the passport that needs the extension, the documentation required (see below), and two new 2 inch by 2 inch (5 cm x 5 cm) passport photos.

Since there are various reasons for limiting a passport, a case by case examination must be made by the passport section of the Embassy before determining whether a replacement passport can be issued. Often the passport can be replaced only after authorization is received from the Department of State in Washington, D.C. Full validity passports are printed only in the U.S.

Replacing Limited Validity Passports Issued for Emergency Travel

Limited Validity passports that cannot be extended are issued for a variety of reasons, but usually are designed to cover emergency travel. Such passports, valid for a maximum of twelve months, can only be replaced with a full validity passport.

To obtain a new full validity passport you should apply using the "U.S. Passport Re-Application Form"(DS-5504) along with the limited validity passport, the letter which you should have received at the time you obtained the limited validity passport, and two new 2 inch by 2 inch (5 cm x 5 cm) passport photos. Please follow the requirements as explained here.

If you trade in your limited validity emergency passport before it expires, there will be no fee to have it replaced with a full validity passport. If you wait until after the limited validity passport has expired, you must use form DS-82 (or DS-11 as appropriate) and you will be charged the full fee to have it replaced. The fees for a replacement passport are $75 for adults and $85 for children.

We cannot issue a second limited validity passport to replace an earlier one; you are expected to provide the information needed to enable us to issue a full validity passport following the issuance of a limited one.

Because of these potential delays, you should not make any unchangeable travel plans until we are able to hand you your new passport.
